What you're reporting
A service charge was added to the card machine without you being asked. (We only track this one practice — see about for why.)
The name on the sign outside — e.g. "The Hawley Arms".
Use the date you were there. Must be within the last 5 years.
By submitting you confirm this happened and is accurate to the best of your knowledge. We log a hashed token (not your IP) to prevent abuse.
